Documents Required to Apply for Membership
- Completed two page Application Form
- Original Transcripts
- Verification of Practicum/Internship Hours (for applicants who graduated after May 1, 2011) - Download Practicum/Internship Reporting Form
- Completed first page of Application Form
- Photocopy of Student ID
The Membership Review Committee must approve all Professional Memberships. The committee generally meets during the last week of each month to review all applications received that month. If membership has been approved, it will generally be processed within two weeks of the committee meeting. If there are any issues with the membership, the Membership Review Committee will contact the applicant for further information.
