Registered Professionals:

• Completed two page application form  - Download Application Form
• Original Transcripts
• Verification of Practicum/Internship Hours (for applicants who graduated after May 1, 2011) - Download Practicum/Internship Reporting Form
• Fees

Students:

• Completed first page of Application Form - Download Application Form
• Photocopy of Student ID
• Fees
